This tool is a semi-quantitative paper based food frequency questionnaire, which includes up 162 food items and an open section. The questionnaire was developed to be used in a subsequent study which aimed to collect dietary data on patients undergoing coronary artery bypass grafting (CABG) surgery. The tool was validated in adults in 2001.
