Summary

This tool is a paper based food checklist, which includes up to 115 food items. The questionnaire was developed to be a concise, simple tool for use by non-specialists to assess diet in children attending primary schools. The tool is intended to be used prospectively as a tick list record for all foods consumed over one 24-hour period, with a retrospective breakfast section. This tool has been validated in 2006 (3-7 years old) and 2015 (8-11 years old). The tool is available online through the Food Questionnaire Creator.

Instructions on use

Instructions on how to use the dietary assessment tool can be found in the publications and for the FQC version, the instructions will be included in the FQC application.

Administration Method

The tool has three parts. Part 1 - by-proxy: parent or caregiver. Part 2 - by-proxy: trained fieldworker, dinner supervisor or parent classroom assistant. Part 3 - self completion with assistance from the classroom teacher.
