Summary

This tool is used by the School-Based Nutrition Monitoring (SBNM) Project funded by the CDC and USDA. The aim of the project was to design, develop, and evaluate the psychometric properties of surveillance tools to be used in elementary, middle, and high schools. The tool was specifically developed to assess nutrition-related behaviors, attitudes, and knowledge and some physical activity behaviors for children at middle and high school levels. The tool was validated in 2003.

Special Considerations

The investigators selected and/or adapted items from a variety of references and survey instruments to form the questions. Questions were modified for each grade level with consideration for reading level and cognitive ability.
